Check or Money Order: $89.99 made payable to Cole Warner Casting (checks allow 7 days to clear)

Send TWO recent photos. They can be different; only one will be used.

Please Note: Personal information will only be shared with a bonified cleint.  These will not be incluided on the website.  They will be secured in our office.

A close up is preferred.   Please look at current CWK photos to get an idea of what is required. Also, either by email or letter, we will need the child’s, name, height, weight, and date of birth.  Any special skills that they have, like Speaks Spanish, athletic, loves animals, dancing, etc.  And of course your email address, your physical mailing address as well as your phone number(s).

As soon as we have all of the above in our possession, the photo will be put on the Cole Warner Kids Directory website.
